Format of Income Affidavit

Hi, I read the letter to the candidate posted on the jee.iitb.ac.in site.In the scholarship section it mentions
"For the proof of income Form No. 16 or an Income Affidavit (as per attached format) should be submitted."
But i could not find the format in the letter.Please tell me where to find it.

Approach a lawyer. He/she will know the exact format. It just needs to be a letter on a stamp paper stating that you have X income for the financial year and take the requisite stamps.

the form no 16 is for salaried persons... my dad is a buisnessman so wat form do we need to fill... i read in IIT K site that for buisnessmans IT return for last year will do... please clarify

My fathers income is below tax slab . So we dont have ITR returns will income affidavit and income certificate sufficient?
